Clinical Documentation & Assessment Support
At Clear Water Wellness, we recognize that clinical documentation can sometimes be an important part of accessing accommodations, benefits, leave, housing-related supports, or other resources.
Documentation is not simply a matter of completing a form or writing a letter. Depending on the request, it may require clinical assessment, review of relevant history and functioning, collaboration with you regarding your goals, and professional consideration of whether the requested documentation is clinically appropriate and supported by the available information.
Our clinicians do not complete documentation simply because it has been requested. Any documentation we provide must be consistent with our clinical findings, professional scope, ethical responsibilities, and the information available to us.
Understanding Your Options
You do not have to obtain clinical documentation through Clear Water Wellness. We are one of many providers who may be able to provide clinical documentation when it is appropriate and within their professional scope. Depending on the type of documentation being requested, a qualified professional may include a physician, psychiatrist, psychologist, nurse practitioner, physician assistant, licensed clinical social worker, licensed professional counselor, or another appropriately licensed healthcare or mental health professional acting within their professional scope.
If you already have a primary care provider (PCP), therapist, psychiatrist, or another healthcare professional who knows you and your history, it may make sense to start by talking with them about what you need. A provider who is already familiar with your care may have enough information to determine whether the requested documentation is appropriate.
Keep in mind that different types of documentation may require different professional credentials, expertise, or forms of evaluation. Not every provider is qualified or authorized to complete every type of documentation. If you are requesting documentation for a specific purpose, we recommend confirming what type of provider, evaluation, or credentials are required by the organization requesting it before beginning the process.
A healthcare professional's role is to evaluate and document clinical information when appropriate. This means a provider cannot guarantee approval of a request for an employer, school, housing provider, government agency, or other organization.
Our goal is to ensure individuals seeking clinical documentation are aware of their options to make this process transparent and clinically responsible, whether you choose to pursue documentation with us or another qualified provider.
What We May Be Able to Help With
Emotional Support Animal (ESA) Documentation
We can provide clinical assessment and, when appropriate, documentation regarding the role of an emotional support animal as part of your support or treatment plan.
FMLA & Medical Leave Documentation
When appropriate and within our clinicians' scope of practice, we may be able to assist with documentation related to mental health needs and medical leave.
Documentation for Supporting Gender-Affirming Care
Some healthcare providers may request clinical documentation as part of gender-affirming care. When appropriate and within our clinicians’ scope of practice, we may provide individualized clinical assessment and documentation related to these requests.
Disability Documentation
We may be able to provide clinical documentation describing mental health-related symptoms, functional limitations, and disability-related needs when appropriate. This may include documentation supporting requests for reasonable accommodations, such as accommodations with school or college, workplace accommodations, disability-related requests.
Fees & Payments
Clear Water Wellness is an out-of-network provider. Clients are responsible for payment of all services related to the assessment and documentation process. All applicable fees will be communicated to you before the documentation process begins.
You will be responsible for:
Individual clinical appointments: Any appointments needed as part of the clinical assessment are billed at the provider’s standard session rate. * Please note, documentation fees are billed separately from psychotherapy appointments.
Documentation fee: Documentation and/or letter preparation are billed at $75 per hour and per document. This fee covers the clinical time required outside of scheduled appointments to review relevant information, complete documentation, and/or prepare the letter when indicated. The documentation/letter fee only applies when documentation is issued. We make every effort to complete documentation as efficiently as possible. However, we do not rush the process at the expense of a thorough clinical review and the amount of time may vary depending on the complexity of the request. You will only be charged for the actual time spent completing the documentation.
Outstanding Balances & Documentation
All outstanding balances must be resolved before clinical documentation or other requested records are released or handed off. This includes any of the aforementioned applicable fees. Documentation will be provided once the account is current and all applicable fees have been paid.
When you schedule with us, you are investing in the clinical evaluation and professional assessment, not purchasing a guaranteed letter, form, accommodation, leave approval, or other outcome. Fees for clinical services and documentation-related work are therefore owed regardless of whether the requested documentation is ultimately approved.

Requests for assistance animal documentation are considered within the context of an established therapeutic relationship.
Because clinical documentation requests involves a clinical assessment and professional judgment, Clear Water Wellness does not provide documentation based solely on a single appointment or an initial intake.
For newer clients, clinicians generally need sufficient time and direct clinical contact to develop an informed understanding of the client's mental health, functioning, treatment needs, and whether the request is clinically appropriate based on the information provided.
The treating clinician may determine that additional sessions or a longer period of treatment are necessary.
The number of sessions alone does not guarantee that documentation will be provided.

The clinician will consider whether the available information supports a clinically appropriate and disability-related request.
The clinician is providing a professional clinical assessment—not making a legal determination about whether you qualify for a disability accommodation.

A disability-related accomodation may be one component of a comprehensive support plan. It is not automatically considered a replacement for psychotherapy, psychiatric care, medication management, crisis planning, or other clinically indicated services.
Depending on your individual needs, your clinician may recommend:
Continued individual psychotherapy
Ongoing mental health treatment
Psychiatric evaluation or medication management, when indicated
Coordination with primary care or other healthcare professionals, with appropriate consent
Coping and emotional regulation strategies
Social and community supports
Crisis or safety planning, when clinically indicated
Additional behavioral health or supportive services
Recommendations are individualized based on your clinical presentation and needs.
You have the right to make your own decisions about whether to pursue, modify, or decline any recommendation we make. Our goal is to work collaboratively with you while maintaining both your autonomy and our professional responsibilities.

After gathering sufficient clinical information, your treating clinician will determine whether documentation is clinically appropriate.
The clinician may determine that:
Documentation is clinically supported
Additional clinical information or treatment is needed before a determination can be made
The request is not clinically supported
The clinician cannot provide documentation due to limitations in scope, knowledge, or the nature of the request
A clinician's decision not to provide documentation does not prevent a client from seeking appropriate legal guidance or discussing their needs with another qualified healthcare professional.

When your clinician determines that documentation is clinically appropriate, they will prepare documentation based on your individual circumstances and the purpose of the request.
Because documentation requires clinical review, preparation, and appropriate professional oversight, please allow approximately 2–4 weeks to receive completed documentation after the clinician has determined that documentation will be provided.
This timeframe is an estimate and may vary depending on the complexity of the request, the information available, and whether additional clinical information or clarification is needed.
Documentation is not a standardized certificate, approval, or guarantee of eligibility. Instead, it communicates relevant clinical information regarding your circumstances, functional needs, and the professional recommendations supported by the assessment.
Depending on the circumstances, documentation may include:
Your name and identifying information
Your clinician's name, credentials, license information, and contact information
The date the documentation was issued
Information regarding the clinician's professional relationship with you
Relevant information about a mental health condition or disability
Relevant functional limitations or disability-related needs
An explanation of the relationship between your disability-related needs and requested accomodation
A clinical recommendation supporting the requested accommodation, when appropriate
Your clinician will only include information that they can clinically support and that is relevant to the purpose of the documentation.
What to Expect
Documentation Is More Than Just a Form. It’s a Clinical Process.
We understand that documentation may be time-sensitive and that you may need a form or letter for a specific deadline. At the same time, we cannot ethically or clinically guarantee a particular outcome or complete documentation that is not supported by sufficient clinical information. Our goal is to make sure that anything we document is accurate, clinically appropriate, and supported by the information available to us.
If you are not currently a client, you will need several appointments to establish a therapeutic relationship, gather relevant information, assess your needs and functioning, and determine whether the requested documentation is clinically appropriate. The number of sessions needed will vary based on the individual and the type of documentation being requested.
How We Practice: Clinical Boundaries & Expectations

When clinically appropriate and within the clinician’s professional scope, Clear Water Wellness may provide documentation that describes your individual clinical needs and how they relate to the request being made.
Depending on the type of documentation requested, this may include information about:
Clinical symptoms and presentation relevant to the request
Functional limitations, including how symptoms or a disability may affect areas such as work, school, relationships, daily activities, or other areas of functioning
Disability-related needs identified through the clinical assessment
The connection between identified clinical needs and the requested support, accommodation, or service
How a requested support or accommodation may help address specific functional limitations or reduce the impact of identified symptoms
The clinical rationale for information or recommendations included in the documentation
Relevant treatment or clinical information when it is appropriate and necessary to address the request
The information included will depend on the purpose of the documentation, the requirements of the organization requesting it, and what your clinician can appropriately support based on their knowledge of your care.

Clear Water Wellness does not:
Offer one-time appointments for documentation requests solely for the purpose of obtaining a letter or documentation.
Certify, register, approve, or guarantee eligibility for a disability, accommodation, leave, benefit, service, or other legal status
Make decisions that belong to another professional or organization, such as determining whether an employer must approve an accommodation or whether a healthcare provider should provide a particular medical treatment
Misrepresent or exaggerate a client's diagnosis, symptoms, functional limitations, treatment needs, or clinical history
Document information that has not been adequately assessed or supported by the clinical information available to us
Backdate documentation to establish a clinical need or circumstance that was not previously assessed or documented
Complete forms or provide letters simply because another person or organization has requested them
Provide documentation solely to help someone obtain a preferred outcome, avoid a fee or restriction, bypass a policy, or meet an administrative requirement
Provide legal advice or determine legal rights, including whether a person is legally entitled to a particular accommodation, leave, benefit, or service
Provide evaluations or documentation that require credentials, training, or expertise outside of the clinician's professional scope
Provide documentation for services or purposes that require a different type of healthcare professional or specialized evaluation
If we determine that we do not have enough information to appropriately complete a request, or that another professional would be better suited to address it, we will communicate that with you and, when appropriate, discuss next steps or referral options.
We Do Not Provide “Documentation on Demand"
Clinical documentation is not provided solely because it has been requested or because an outside party has identified a need for it. Requests for documentation are reviewed based on clinical appropriateness, the available clinical information, the provider’s scope of practice, and applicable ethical and professional standards.
We do not provide documentation simply because:
A client has been told that documentation is necessary to obtain an accommodation, leave, support, or service.
A client believes they qualify for a particular accommodation, benefit, or service.
A form requires completion by a mental health professional.
A client has a deadline or requests that documentation be completed on an expedited basis.
A client has already made arrangements, commitments, or decisions that depend on receiving the requested documentation.
Documentation Is Not Guaranteed
We do not guarantee that a client will receive requested documentation .
A diagnosis alone does not automatically establish a need for an accommodation or request. Similarly, not every client who may experience benefit from a request will meet the criteria for a disability-related accommodation.
Our clinicians must independently determine whether providing documentation is clinically supported.
If the available clinical information does not support the request, the clinician may decline to provide documentation.
This decision is not a judgment about the importance of the client's request. It reflects our responsibility to provide accurate, clinically appropriate documentation based on sufficient clinical information —not to guarantee a particular outcome.
New Clients & Documentation Requests
Are We a Good Fit for What You’re Looking For?
If you are reaching out to Clear Water Wellness specifically because you need clinical documentation, we want you to understand what that process looks like before scheduling. Please understand that this is a process rather than a single appointment or paperwork request. For this reason, we cannot guarantee that documentation will be provided after an initial appointment.
Documentation is not simply a matter of completing a form or writing a letter. Our clinicians need to have enough information about you, your history, current concerns, and functioning to determine whether the requested documentation is clinically appropriate and whether we have enough information to accurately support what is being requested.
Before Scheduling Consider:

If you are already established with a therapist, psychiatrist, PCP, or another healthcare professional who is familiar with your history and current needs, it may make sense to start with them. We are not the only providers who have the capacity to provide clinical documentation.

Different requests may have different requirements. Some may require a particular type of professional, specific forms, or specialized evaluation. We recommend finding out what is required before beginning the process.

Documentation is based on clinical information, not simply on what you would like a letter or form to say. Your clinician may need to learn more about your history, symptoms, functioning, and current needs before determining whether documentation is appropriate.

We understand that documentation can feel urgent, but we cannot ethically rush a clinical assessment. If you are a new client, several appointments may be needed to establish a therapeutic relationship and gather enough information to make an informed determination.

When you schedule an assessment or documentation-related service, you are paying for your clinician's time, assessment, clinical expertise, and professional judgment. You are not paying for a guaranteed letter, form, diagnosis, accommodation, leave approval, or other outcome.
There may be circumstances in which, after learning more about your situation, your clinician determines that the requested documentation is not clinically appropriate, cannot be adequately supported by the available information, or falls outside their professional scope. In those situations, we will be honest with you about our clinical determination.
